Hello and welcome to CBST!

In order to keep our community safe, CBST requires anyone who wishes to enter the building, to be fully vaccinated and able to provide proof.

The CDC considers a person to be fully vaccinated:

• 14 days after their second dose in a 2-dose series, such as the Pfizer or Moderna vaccines, or
• 14 days after a single-dose vaccine, such as Johnson & Johnson’s Janssen vaccine

If you are fully vaccinated, please complete the following COVID-19 self-screening form by 11AM on the morning of your visit. The results will be clear in determining whether you are eligible, or not, to enter the building. The names of those who pass the self-screening will be placed on a list at the door with security.
